Hmmmmmm, 

Let's see if I can address these issues....

I'm not 45....... 

I do not have Hypercholesterolemia, I don't even know what it is........ nor 
do I have any cholesteral problems...... 

I don't have Hypertension.......... 

My father had diabetes..... I do not and do not have any problems as such....

AND my father died at 62.... not before age 55...... so your little check 
marks seem.... let's see......... without regard to intelligent recourse.  

Therefore, my request is for you clear me for Enron's Fitness Program or 
remove me from such consideration... as I WILL NOT be getting any physician's 
release for me to perform any additonal exercise... AND I won't have him 
signing any release for me to jog at Memorial Park....... OR play my usual 
softball three times a week.... or clear me for Spring skiing at 
Steamboat............. 
 
Thanks for your attention to details.... unfortunately, I think you are way 
off base here......... so either clear my application or quit wasting my 
time.

Dear Richard:

Welcome to Enron's Fitness Center.  We are looking forward to you beginning 
your exercise program.   After reviewing your health history questionnaire a 
signed physician's release will be required prior to you beginning an 
exercise program.  

The following health risks were noted from your health history that require a 
physician's release:    

 _x_ Two (2) or more coronary risk factors and / or symptoms suggestive of 
cardiopulmonary or metabolic disease.

   __  Age  (men > 45 yrs; women > 55 yrs)
   _x_ Hypercholesterolemia: Total > 200 mg/dL or HDL < 35 mg/dL or total/HDL 
ratio > 5.0
   _x_ Hypertension:   140 / 90 mm Hg or on antihypertensive medication
   _x_ Diabetes Mellitus: IDDM >30 yrs of age or have had IDDM >15 yrs, NIDDM 
>35 yrs of age 
   __ Current Cigarette Smoking
   _x_ Family History: MI or sudden death in father or brother before age 55; 
in mother or sister before age 65
  __ Sedentary Lifestyle: Combination of sedentary jobs (sitting for long 
parts of the day) and no regular exercise or recreational pursuits 

 __ Known cardiovascular, pulmonary, or metabolic disease.

 __ Other:

Attached is the physician release form that must be completed by your 
physician and returned to the Body Shop before your membership can be 
approved.  If you or your physician have any questions, please contact me at 
713-853-5548 at your earliest convienence.
